Under the Hearing Services Program (HSP) the system and benefits are the same no matter which provider you choose, the difference is the expertise and quality of service between providers!
Eligibility for the Hearing Services Program

H.S.P. (Hearing Services Program) Voucher scheme eligibility
You are eligible for the voucher scheme if you are an Australian citizen or permanent resident, are aged 21 years or older, and are:
(A Commonwealth Seniors Health Card does not provide eligibility for the program.)
a pensioner concession card holder or their spouse
a Department of Veterans’ Affairs (DVA) Gold Card holder or their spouse
a DVA White Card holder (hearing specific conditions) or their spouse
a member of the Australian Defence Force
referred by the Disability Employment Service.

Please supply your CRN no. when completing this form and use the same spelling of your name as on your D.V.A. or Pension card so we can verify your eligibility.
